Service Request

If you are experiencing a problem with one of our products, please fill out and submit the following Service Request Form       

For suggestions, or requests for improvements, please use our suggestion form.

Service Request Form

When you submit your service request report, it will be entered into our database for tracking purposes.  Periodically we will post notices regarding service requests reported by our customers, and tips or workarounds.     

Please be as specific as possible when describing the problem.       

  1. What Medical Billing Software are you currently utilizing?

    (example: "How do I install E-MedOutcomes on a Network?")

We are working on additional interfaces when individual physicians request these.

  • Billing Software interfaces--Please send your request along with the file structure of the billing program patient list and physician list files i.e. (patlist.dat, refdoc.dat on Server/C:) Where the server or workstation which houses the billing software program is specified along with the drive letter associated with the patient and physician list files.
