This program allows Validated Outcomes Measures to be collected from patients without the need for paper forms or expensive and time consuming office support staff.  The program runs on a Computer "Kiosk" allowing patients to navigate to the correct form and input their data utilizing a touch screen and an intuitive patient-computer interface.  Patients need no computer experience in order to easily enter data reliably.

All data is complete!  Navigation is one screen per question, so that the only way to finish a questionnaire is to complete it. 
When the patient is done, the form is automatically scored and a histogram of the scores is viewed on the same screen.  This can be printed for the patient to review along with their prior scores.  This allows patients to participate and understand their treatment completely.
Data is stored in SQL format, and the program is ODBC compliant to allow connections to be made to other databases such as Electronic Medical Records.

Patient demographics are imported from native office billing systems thereby eliminating duplicate data entry and providing a means for secure patient log-on.

Data is stored in tabular format minimizing file space and eliminating security or HIPAA concerns.

Form Designer© Form Designer©  is a robust development tool which can be used by the novice or professional alike.  It allows the physician or researcher to develop questionnaires for patient use, utilizing a simple word processor-like architecture.  Questions and Answers alike, are entered by simple typing, while E-MedOutcomes® converts these to an interactive interface.  This runs the questions interactively with the patient, and stores the resulting data in an SQL database.  Criteria may be entered for form and question navigation, and audiovisual files may be easily created and linked to each question and answer so that they are read to the patient while they complete it.  While using the Form Designer®, the question, entire questionnaire or all of the questionnaires may be previewed in E-MedOutcomes®, so that the actual working effect may be appreciated during the design process.                                                     Form Designer Screen Shot Form Designer© may also be used as an intuitive interface for patient input of demographic and medical information.  This may lessen the burden of patient data input, and the otherwise necessary burden of collection and verification of this data.  In this way, we may work toward improved patient care while lowering office costs. Form Designer Document (PDF) E-Outcomes Administration© E-Outcomes Administration©  is a powerful data mining tool which gives the physician or researcher the ability to query their local data to see the effect of treatment on results.  Data can be easily evaluated in table or histogram format, and several queries can be run on the same screen in order that comparisons and temporal progression evaluated.  This is an intuitive interface with easy to use tools for data mining.  We have built in simple average value and standard deviation operations which may be applied to a specific data set for ease of evaluation.  Alternatively, more complex score files may be developed in simple text editor format.  In this way, the advanced user may customize the data reports which they would most like to see.  At the same time, this has no affect the data that has been collected locally, allowing for combined physician data to remain unimpeded.                                                       e-Outcomes Administration Screen Shot e-Outcomes Administration Document (PDF)
